Download the FollowMyHealth App, which is the patient portal that we use to send required paperwork, and where you will submit copies of your insurance card(s) and Identification (front and back of both). You will also be able to communicate with your provider and our adminstrative team members via the portal in a secure, HIPAA compliant manner.
